Facet Injections as Pain Management Treatment for Back Pain from Facet Arthritis

Low Back Pain afflicts 90% of Americans at some point. The vast majority of it, 90%,  will resolve within 6-12 weeks no matter the treatment. And what about the ten percent that becomes chronic??

If the back pain is being caused by facet arthritis, otherwise known as facet syndrome, the back pain can be disabling and chronic.. Facet joints connect the bones of the spine, called vertebrae, and allow for spinal movement.. The lumbar region of the spine contains five vertebrae and is located in the lower back.

Lumbar facet joints are named for the vertebrae they connect and the side of the spine where they are found. Typically, pain being generated from the facet joints causes back pain and does not radiate into the legs.. Lumbar facet joint pain results from either injury to the facet joint cartilage or to the connecting ligaments surrounding the facet joint.. This can be from degenerative arthritis or a result of post traumatic injury. Facet joint pain may range from simple pain such as simple muscle tension to more severe disabling pain.. Depending on which of the facet joints is affected, back pain may radiate down to the one’s buttocks from the affected joint..

Common tests such as x-rays or MRI’s may not always show if a facet joint is the reason for your pain.

One of the treatment options for low back pain from facet arthritis is facet injections, also known as facet blocks.. Pain doctors perform facet blocks commonly for back pain from facet syndrome. The injections can give back pain relief for weeks to months, and are performed as an outpatient minimally invasive pain management procedure.

Facet injections are performed with appropriate local numbing anesthetic medication.. A facet joint block can be both diagnostic and therapeutic for back or neck pain. A facet joint injection can tell the physician that the facet joint is indeed the cause of the pain and can help relieve the pain and inflammation.

Fluoroscopy, a form of x-ray, is usually utilized to ensure the needle is properly placed into the facet joint and dye is then placed to make sure the needle is accurately in the facet joint.

Once the needle is placed in the facet joint, the pain doctor injects numbing medicine along with a corticosteroid.. The numbing medicine wears off within a day, and then the steroid medication begins to work in a few days. The pain may be sustained for days, weeks, or months, or it may not work at all. Depending on the location of pain, one or more injections may be administered. If pain relief occurs from the injection, this highly suggests that the facet joint (or joints) injected were causing the back pain..

Pain relief occurs in three to ten days as the steroid medication reduces inflammation. As many as three injections per facet joint may be administered per year..
